Change management reporting for this project (and maybe even project performance measurement) are under the authority of the appropriate operational team; the change management team will have made recommendations based on a performance self-assessment and experiential learnings.
All projects end and eventually all good projects take steps to close out and hand over the changes to operational authorities. This should happen seamlessly. In tandem, capturing the experiences and lessons of the project for the future is just good sense. The work to close out the change management “project,” including hand overs, post mortem (self-)analysis, and Lessons Learned reporting is the essence of this Play.
This activity is the structured transition of responsibility and accountability for outcome metrics to the previously determined operational authority.

This action is to produce a formal change management-specific lessons learned report and record for the project.
